Job Summary
The SUD Patient Navigator provides outreach, education and resource services to clients at Sea Mar’s outpatient Behavioral Health sites and in the community, collaborating with therapists, case managers, and clinical staff to assist clients in accessing housing, income, employment, medical and mental health treatment, and meaningful community activities. Responsibilities include delivering outreach to individuals with substance use disorders, offering case management services (including documentation and coordination with Medicaid/insurance), educating clients about recovery, navigating community resources, and supporting treatment plans and discharge planning while maintaining confidentiality and professional standards. Requires active WA peer counseling license within 90 days, certification, TB screening, and annual competency testing; 1 year of relevant experience; and ability to work in a team-focused, culturally competent environment in Washington State.
Required Qualifications
- Must be a consumer of service within Washington State’s mental health system or have experience working in the Substance Abuse field.
- Must be a Certified Peer Counselor in the State of Washington or have experience working with SUD clients.
- Must hold and maintain an active Washington State Agency Affiliated Counselor License within 90 days of hire date or demonstrate experience working with the population served.
- Must have and maintain a current TB test.
- Must complete an annual clinical competency test as a SUD Patient Navigator.
- Education and Experience: One year paid or volunteer experience working with Mental Health and graduation from the Washington Institute Peer Advocacy Training Program or equivalent program required.
- Must be a resident of Washington State or otherwise eligible to work in the United States.
- Must be able to read, write, and speak English effectively; ability to work with diverse populations.
- Must have and maintain a valid driver’s license.
