Job Summary
SUD Counselor to perform assessment and identification of client needs, coordinate development of individualized treatment plans, facilitate outpatient group and individual counseling, and coordinate discharge planning. Responsibilities include meeting with family/significant others as appropriate, coordinating treatment plans with identified problems, goals and methods, documenting progress notes, conducting screening/evaluation of substance use with treatment recommendations, teaching coping techniques to prevent relapse, providing court progress reports, helping clients build support networks, tracking recovery, counseling clients in groups or one-on-one, collaborating with other professionals on treatment plans, and complying with organizational policies. Minimum qualifications include a Master’s degree in a Human Services field and relevant credentialing (QMHP or QAP) with credentialing options such as CADC/CRADC/CRAADC/CCJP/RADC family and related qualifications.
