Job Summary
Provide navigation and linkage services to individuals seeking substance use and behavioral health support; work as part of a team under the direction of the Program Manager to connect individuals to services, provide ongoing support, and coordinate care. Responsibilities include answering calls, identifying service needs, connecting individuals to resources, following up to ensure access to care, offering guidance and advocacy, maintaining relationships with community agencies, supporting outreach, documenting interactions per program requirements, maintaining HIPAA confidentiality, collaborating with team and leadership, and performing additional duties as assigned.
Required Qualifications
- High school diploma or GED required
- Strong communication and interpersonal skills
- Ability to work in a fast-paced, team-oriented environment
- Basic computer skills and ability to learn internal systems
- Valid driver’s license, reliable transportation, and active auto insurance
