Job Summary
Conduct initial and follow-up assessments to determine treatment needs and provide weekly individual counseling sessions. Facilitate individual, group, and family therapy sessions while collaborating with clients to develop personalized treatment and case plans. Document progress, monitor outcomes, and communicate updates to the clinical team, participating in weekly meetings and developing aftercare plans. Act as a liaison with insurance companies and probation officers, represent the organization in court, and transport clients as needed. Compile documentation for audits and coordinate with other departments to ensure seamless care. Maintain licensure and attend required trainings.
Required Qualifications
- LCDC II, LCDC III (must remain active and valid)
- Valid driver's license
- Associates or Bachelor's degree in Health Services, Social Work, Counseling, or related field
- Strong organizational and time management abilities
- Effective communication and interpersonal skills
- Proficient with technology and electronic health record systems
- Ability to remain professional and empathetic in demanding situations
- Schedule: 8-4 M-F working one to two weekend days a month
