Job Summary
Manage the full Individual Development Plan (IDP) cycle for associates, including assessment, planning, implementation, monitoring, and evaluation of their accommodation, retention, and career growth needs. Provide comprehensive single-point-of-contact support through life and work coaching, personal counseling, work assessments, mediation, and advocacy with organizational and community resources. Coordinate case management services, pre-schedule meetings, and refer associates to external training, social aids, and local programs to foster continued growth. Document all coaching, appointments, and activities in electronic case management systems while maintaining accurate records and adhering to safety regulations. Intervene in crises as needed and travel to multiple locations to serve the assigned area, ensuring associates achieve self-sufficiency and meet retention goals.
Required Qualifications
- A high school diploma or equivalent
- Must maintain a high level of confidentiality
- Must have reliable transportation
- Must be punctual and have dependable attendance
- Must conduct work in a safe manner
- Ability to sit, stand, bend, and reach
- Ability to lift, push, pull, up to, 25 lbs
Desired Qualifications
- A minimum of one (1) year of experience in case management
- Extensive knowledge of community resources in the four sector areas: business, government, non-profit, and faith-based organizations
- Experience as a facilitator, instructor or career coach
- Good computer skills (proficient in Microsoft Office)
- Excellent written and verbal communication skills
- Ability to communicate and interact well with a variety of personality types
- Ability to analyze opportunities and problems, identify and evaluate alternatives, and develop effective approaches to address any issues
- Ability to plan, implement and evaluate the achievement goals, objectives, and work plans
