Job Summary
Provide nursing care to adults with disabilities in an adult day health care center; perform health monitoring and nursing services, coordinate admissions/discharges, schedule in-house medical visits and transportation, maintain participant medical records, support quarterly care plan meetings, provide clinical support and education to participants and family/caregiver teams, and assist with activities of daily living and emergency situations in a person-centered setting.
Required Qualifications
- Graduate of an accredited nursing program
- Current Florida Licensed Practical Nurse License
- At least one year of experience working with individuals with disabilities and/or aging population and Alzheimer's patients
- Ability to assist with transfer of participants to and from chair or wheelchair and assist clients with activities of daily living
- CPR and first aid certification a plus, but can be obtained/renewed once hired
