Job Summary
Identify patients with substance use disorders, offer brief screening and motivational encouragement, and use a prevention and risk reduction approach to assist them accessing Narcan and medication for addiction treatment. Coordinate referrals, arrange follow-up appointments, and assist with discharge planning to reduce gaps in care while working alongside clinicians, behavioral health, and community partners. Remove barriers and strengthen continuity of compassionate, patient-centered support for marginalized populations.
Required Qualifications
- Nonjudgemental, energetic, positive approach to SUD support
- Interest/proficiency in working with marginalized populations
- Ability to communicate with patients clearly, respectfully, and in a culturally appropriate manner
- Ability to use a computer and basic office equipment
