Job Summary
Lead and deliver PACT’s Substance Use Disorder programming as a SUDP, dividing time around ~80% program facilitation and ~20% direct case management. Update monthly treatment plans based on clients’ stage of change, provide evidence-based interventions, act as subject matter expert on SUD, train staff on SUD topics, and conduct individual and group treatment in office and community settings using a stage-based, non-confrontational model. Plan and facilitate groups, coordinate harm reduction supplies, advocate for client access to resources, foster collaboration with DESC and external partners, participate in supervision, clinical reviews, and on-call rotations, and support living conditions and unit mitigations for clients. Maintain clinical records, participate in QA, and engage in ongoing multidisciplinary collaboration to support clients with dual diagnoses. This role operates within DESC’s PACT team, emphasizing integrated supports across treatment, housing, and community resources, with a schedule aligned to days (Office Day shift) and weekend/holiday coverage.”,
