Job Summary
The Substance Use Disorder Counselor collaborates within Carle Health to provide evaluation, counseling, and therapeutic services to chemically dependent patients and families (inpatient and outpatient). Responsibilities include developing individual treatment plans, acting as a liaison with referral sources and families, coordinating care with the interdisciplinary treatment team, supporting patients through treatment and aftercare planning, addressing court-ordered referrals, maintaining documentation, and contributing to ongoing professional development and community education. Required Qualifications
- Bachelor's Degree in Social Work (within two years if pursuing LSW license)
- Certification or licensure options within 2 years: CSADC, CADC, LSW, LCPC, CRADC, CAADC, LPC
- Basic Life Support (BLS) within 30 days
- Driver's License - Illinois
- Multicultural sensitivity and English proficiency
- Proficiency in Microsoft Office and basic computer skills
