Job Summary
Screen, triage, and conduct crisis planning for clients to facilitate seamless access to evidence-based treatment, medication-assisted therapy, and recovery supports. Provide case management, service coordination, and supportive counseling while reducing barriers to care and protecting client rights. Complete clinical and administrative documentation for billing and records, and maintain regular face-to-face reviews to assess progress and modify service plans. Collaborate with Universal Access staff, crisis teams, and community agencies to coordinate interventions and ensure continuity of care.
Required Qualifications
- Bachelor's degree in Social Work, Psychology, or related Human Services field
- Associates degree in Human Services field with two years of case management experience
- Must Roster under the Office of Professional Regulations
- Addiction Apprentice Professional (AAP) Credential
- Valid Driver's License
- Excellent verbal and written communication skills
- Excellent interpersonal and customer service skills
- Excellent organizational skills and attention to detail
- Excellent time management skills
- Strong analytical and problem-solving skills
- Ability to prioritize tasks and to delegate them when appropriate
- Proficient with Microsoft Office Suite
- Basic understanding of clerical procedures and systems such as recordkeeping and filing
- Ability to work independently
- Knowledgeable about ASAM levels of care and Vermont system of care
- Competencies in screening, assessment, engagement, treatment planning, collaboration, referral, motivational interviewing, HIPAA and 42 CFR Part 2, and other evidence-based counseling and professional ethics theories
