Job Summary
Substance Use Case Manager supports families affected by parental substance misuse by conducting screenings, coordinating referrals to assessment and treatment, and addressing barriers to engagement (transportation, childcare, stigma). Collaborates with DCF staff to improve well-being and permanency outcomes, delivering services across DCF offices, community spaces, and clients' homes. Responsibilities include linking families to treatment services, liaising between the treatment system and DCF Family Services, and providing consultation and training. Requires at least an associate degree (bachelor's preferred); certification as Apprentice Addiction Professional or ability to obtain within first year; proficiency with technology (Microsoft Office, EHR, Zoom/Teams); valid VT driver’s license with reliable transportation; and willingness to travel within the state. The role emphasizes building strong relationships across multiple communities and teams, maintaining client confidentiality, and adhering to state and federal regulations. Required Qualifications
- Minimum of an associate degree required
- Bachelor's degree in social work, counseling, or a human services field is preferred
- Experience providing case management to persons in a substance use, recovery, or mental health treatment setting preferred
- Apprentice Addiction Professional (AAP) certificate or ability to test for certification within first year of employment
- Ability to adhere to strong professional boundaries and maintain client confidentiality in line with state and federal regulations
- Valid VT Driver’s License and access to reliable transportation; occasional statewide travel may be required
- Experience with technology including Microsoft Suite and Electronic Health Records; experience with Zoom/Teams
