Substance Use Care Coordinator - Pantops

The Substance Use Care Coordinator enhances patient management and coordinates the patient care cycle for outpatient treatment of opioid use disorder. Responsibilities include coordinating and providing safe, timely, effective, efficient, equitable, and patient-centered care; helping patients achieve wellness and autonomy; facilitating multiple care aspects (case coordination, information sharing, etc.); advocating for patients regarding clinical status and treatment options; developing effective working relations with medical and clinical teams throughout the case management process; arranging abeyances with social services, health and governmental agencies; and complying with State Licensure Standards, CARF Standards, 42 CFR and HIPAA standards. Requires a Bachelor's Degree in a related healthcare field or social services, experience in case management, proficient PC skills, and excellent oral and written communication skills.
