Substance Use Care Coordinator - Pantops

The Substance Use Care Coordinator will enhance the quality of patient management and coordinate care across case management activities to maximize patient satisfaction and cost effectiveness. Responsibilities include coordinating and providing safe, timely, effective, efficient, equitable, and patient-centered care; helping patients achieve wellness and autonomy; facilitating multiple care aspects (case coordination, information sharing); guiding patients to informed decisions about clinical status and treatment options; developing effective working relations with medical and clinical teams; arranging abeyances with social services, health and governmental agencies; and ensuring adherence to State Licensure Standards, CARF Standards, 42 CFR and HIPAA and company policies. Requires a Bachelor's degree in a related healthcare or social service field and experience in case management; strong communication skills and PC proficiency. The role is based in Charlottesville, VA, with in-person duties at the Charlottesville, VA location.
