Job Summary
Substance Use Care Coordinator leading the MAT program at Space Coast Health Center, serving as the primary MAT program case manager, coordinating care from hospital referrals, developing initial care plans within 30 days, documenting plans and progress, and providing comprehensive care coordination and psycho-education to individuals with substance use disorders. Responsibilities include rapid referral response, treatment-plan development and updates, ensuring chart integrity and timely assessment updates, advocacy for persons served, collaboration with a multidisciplinary team, and participating in on-call coverage with evening/weekend rotations. Requires a Bachelor’s degree in a related field and at least two years of experience in community-based treatment; valid driver’s license is required.

- Bachelor's Degree from an accredited university or college in psychology, social work, counseling, or related human services field; at least two years of relevant community-based treatment experience; valid driver's license
