Job Summary
Conduct individual sessions, facilitate group counseling, and deliver didactic educational presentations to support patients recovering from opioid and illicit substance addictions. Complete comprehensive behavioral health assessments, develop individualized treatment plans, and evaluate treatment effectiveness through ongoing client assessment. Coordinate care with community-based providers, manage caseloads in compliance with regulatory standards, and maintain thorough client records. Include family and natural supports in treatment plans while assisting in screening and coordinating primary care services. Maintain knowledge of new developments in substance use fields and serve as the primary point of contact for case coordination.
Required Qualifications
- Associates degree or above
- Active LCDC, LPC or LCSW license
