Job Summary
Conduct individual sessions, facilitate group counseling, and deliver didactic educational presentations to support patients recovering from opioid and illicit substance addictions. Maintain and manage caseloads in compliance with regulatory standards while completing comprehensive behavioral health assessments and individualized treatment plans. Evaluate treatment effectiveness through ongoing client assessment and coordinate care by managing referrals, securing services, and maintaining thorough chart compliance. Include family and natural supports in treatment plans per client preference and serve as the primary contact for communication with other systems and community providers.
Required Qualifications
- Associates degree or above
- Active LCDC, LPC or LCSW license
