Job Summary
Conduct individual sessions, facilitate group counseling, and deliver didactic educational presentations to assist patients recovering from opioid and illicit substance addictions. Complete comprehensive behavioral health assessments, individualized treatment plans, and ongoing client evaluations to ensure regulatory compliance and modify service plans as needed. Coordinate care with community-based providers, manage caseloads, and maintain thorough client records while including family and natural supports in treatment. Serve as the primary point of contact for clinical communication across systems and assist in screening primary care services. Maintain knowledge of new field developments and uphold collegial relationships with professionals. This outpatient role supports clients in a CARF-accredited setting dedicated to addressing the opioid epidemic.
Required Qualifications
- Associates degree or above
- Active LCDC, LPC or LCSW license
