Substance Abuse Case Manager

Provide case management and care coordination for patients enrolled in treatment, ensuring optimum utilization of resources and compliance with external agencies. Coordinate patient assessments, treatment planning, and crisis intervention services while facilitating linkages to community resources for housing, medical, and vocational needs. Conduct individual and family educational sessions, manage discharge activities, and assist patients in accessing financial assistance and daily living support. Adhere to organizational policies regarding confidentiality, attend team meetings, and complete required training courses. Coordinate issues relevant to the team and its relationship within the facility.
