Job Summary
Substance Abuse Case Coordinator II provides comprehensive case management for clients referred to AFF, including leading groups, engaging clients within 24 hours, coordinating assessments, developing service plans, and facilitating collaboration with treatment teams; telecommuter position with evening/weekend on-call as needed.
Required Qualifications
- High School Diploma or equivalent (minimum 3 years related experience)
- Ability to work with multi-problem families in substance abuse, child welfare, or behavioral health
- Associate’s degree preferred in human services/behavioral health
- Valid AZ driver’s license and transportation
- Fingerprint clearance or ability to obtain
- 21 years of age or older to drive on behalf of Catholic Charities
- Proficiency with Microsoft Office
