Job Summary
Telecommuter Substance Abuse Case Coordinator II provides comprehensive case management for AFF participants, engages clients within 24 hours via home visits or calls, orchestrates assessments and service plans, coordinates treatment and community referrals, facilitates group sessions, and prepares documentation and progress reports while supporting families to overcome substance abuse barriers and achieve permanency and self-sufficiency.
Required Qualifications
- High School Diploma or equivalent
- at least 3 years of related experience including working with multi-problem families in substance abuse, child welfare, or behavioral health
- Associate’s degree in a human service or behavioral health-related area preferred
- Knowledge of human behavior, social work theories, community resources
- Counseling, interviewing, assessment, therapeutic relationship skills
- Fluent in Microsoft Office Applications (Word & Outlook)
