Substance Abuse Case Coordinator II

Job Summary

Receive referrals and engage clients within 24 hours via home visits or telephone contact, completing documentation for all interactions. Conduct continued outreach through calls, home visits, and ongoing communication with the substance abuse treatment team. Coordinate and review collateral information including social histories, judicial records, and treatment data. Develop individualized case management plans and service plans, making community referrals and updating plans bi-annually. Facilitate Intensive Outpatient and Outpatient groups using evidence-based curriculum. Complete required assessments tools and submit monthly progress reports and billing. Participate in Adult Recovery Team meetings with multiple stakeholders. Perform other duties as assigned. This telecommuter role supports families addressing substance abuse to achieve permanency and self-sufficiency.

Required Qualifications

  • Minimum of a High School Diploma or equivalent
  • At least 3 years of related experience
  • Experience working with multi-problem families
  • Field of substance abuse
  • Field of child welfare
  • Field of behavioral health
  • Knowledge and understanding of human behavior
  • Knowledge and understanding of social work theories and techniques
  • Knowledge and understanding of community resources

  • Level One Fingerprint clearance
  • Ability to obtain fingerprint clearance
  • Ability to maintain clearance
  • Age 21 years or older

Desired Qualifications

  • Associate's degree in a human service or behavioral health-related area of study
