Job Summary
Conduct educational programs and coordinate skill development for Sub-Acute personnel while ensuring orientation and in-service compliance with facility, state, and federal regulations. Plan new hire and annual competency assessments, maintain staff schedules, and provide patient care aligned with hospital policies and regulatory standards. Operate equipment safely, utilize AIDET and SBAR communication protocols, and collaborate across disciplines to manage patient transfers, discharges, and referrals. Report equipment or environmental issues, complete monthly training, and participate in performance improvement initiatives. Work a 3pm-11pm shift, five days per week, with potential weekend availability and float authority to other hospital areas.
Required Qualifications
- Minimum of two (2) years of direct patient care as a Registered Nurse (RN) in long term care facility
