Sub Acute Coordinator

Job Summary
Oversee daily operations of Chapman Care Center's 33-bed Sub-Acute Unit; coordinate interdisciplinary care with nursing, respiratory therapy, rehabilitation, physicians, and ancillary departments; monitor admissions, transfers, discharges, and census; participate in clinical rounds and care planning; ensure regulatory compliance and support quality improvement; review clinical documentation for accuracy and compliance; serve as a resource for staff on Sub-Acute protocols; assist with survey preparedness and timely admissions; foster strong relationships with patients, families, physicians, hospitals, and community partners.
Required Qualifications
- Current California RN license preferred; LVN with strong Sub-Acute experience may be considered
- Minimum of two (2) years of experience in skilled nursing, Sub-Acute, or post-acute care
- Experience coordinating care for medically complex patients preferred
- Knowledge of Medicare, Medi-Cal, skilled nursing, and Sub-Acute regulations
- Strong organizational, leadership, and communication skills
- Ability to work collaboratively with interdisciplinary teams
- Proficient with electronic medical records and healthcare documentation systems
Desired Qualifications
