Job Summary

Street Outreach Case Manager engages individuals and families experiencing homelessness in the community, connects them to needed services, and provides non-clinical support and advocacy. Works in the field and at the Drop-In Center using a client-centered, strength-based approach to identify needs, set goals, and connect individuals to appropriate resources. Responsibilities include conducting street outreach, performing initial needs assessments, developing individualized service plans, coordinating referrals to medical health, housing, benefits, and other social services, advocating for access to services, assisting with activities of daily living, following up with providers, participating in case conferences, responding to crises with de-escalation and safety measures, documenting via EHR and funders’ databases, ensuring confidentiality, and collaborating with external providers. Requires strong interpersonal and communication skills, ability to work in fast-paced environments, and willingness to travel within the Chicago area. Education/experience requirements are flexible across Bachelor’s, Associate’s, or high school with extensive relevant experience; bilingual abilities may be required; may require a personal vehicle for travel.

Required Qualifications

  • Bachelor’s degree in social work, Human Services, Public Health, Psychology, or a related field — minimum 1 year of relevant experience required
  • Associate's degree in a related field — minimum 2–3 years of relevant experience required
  • High school diploma or GED equivalent required. A minimum of 4–5 years of demonstrated experience in case management, social services, community health, or a related human services field may be considered in lieu of a college degree.
