Job Summary
Provide direct care and assist patients with activities of daily living (ADLs) per their care plans. Act as part of the nursing care team to promote excellent care delivery and document all care interactions in the electronic medical record. Provide feedback to help individualize resident care plans while ensuring the needs of residents and their families are met during the care delivery process. Work in a clean, state-of-the-art skilled nursing facility serving a diverse population, including 90% of short-term patients recovering to return home.
