Job Summary
Deliver intensive therapeutic services using a trauma-informed care approach to individuals, siblings, or families in a home setting. Provide assessment services, caseload consultation, and discharge planning through individual or group counseling sessions. Complete accurate clinical documentation, treatment plans, and progress notes consistent with agency standards and Medicaid regulations. Formulate and implement treatment plans while convening team meetings and facilitating transitions for clients. Perform case management by communicating with partners including the Department of Children and Families, attorneys, and medical personnel. Attend scheduled psychiatric appointments, emergency coverage, and required training. Work flexible hours including evenings and weekends while participating in continuous quality improvement activities.
Required Qualifications
- Master's Degree from an accredited university or college in the field of social work, counseling, or psychology
- Two years' experience in providing services to children, or special needs youth, at risk youth or persons with serious mental illness
- Training in evaluations and assessments
- Training in treatment planning
- Training in treatment interventions
- Training in documentation
- Training in abuse regulations
- Training in patient rights
- Training in crisis intervention
- Valid Florida driver's license with no record of criminal driving offense of license suspension in the past 5 years
- Means of providing job related transportation
- Proof of required insurance
- Insurance eligibility under CHN's current auto insurance policy
- Successful completion and maintenance of FDLE clearance
- Federal background check
- State criminal background check
- Sexual predator screening
- NAPPI Certification or other de-escalation training as assigned
- Current CPR and First Aid Certification
- Ability to communicate effectively, verbally and in writing
- Ability to establish effective working relationships with clients, co-workers, supervisors and other individuals
- Effective interpersonal skills
- Ability to work flexible hours, including evenings and weekends
- Knowledge of agency's organizational structure, standard operating procedures, and policies
- Knowledge of Department of Children and Families rules and regulations regarding the care of children
- Knowledge of child abuse or neglect reporting procedures and methods
- Knowledge of counseling, intervention and treatment methods, strategies and techniques
- Knowledge of quality documentation as required by agency standards, rules, regulations, and Medicaid BHOS
- Ability to conduct assessments, treatment evaluations, counseling in group and individual sessions, treatment planning, and discharge planning
- Ability to work as a multi-disciplinary team member in a positive productive manner
- Ability to convey ideas, concepts and case studies to clients, other clinical staff, families, court, and to other audiences as required
- Ability to participate in play therapy activities
Desired Qualifications
- License in mental health or clinical social work preferred
