Job Summary
State Registered Nursing Aide provides personal care services under the direction of a registered nurse in patients' homes per the IDT care plan. Responsibilities include routine nursing procedures under RN supervision, vital signs monitoring, discussion of visit frequency with the RN, supplies replenishment, use of medical equipment with proper body mechanics, document care, and participate in meetings. Must maintain universal precautions and compliance with all policies, procedures, and regulations; may assist with housekeeping and on-call duties. Requires Home Health Aide certification and completion of a state-approved CNA course with at least one year of patient-care experience; strong communication, independence, and reliability are essential.
Required Qualifications
- High School graduate or equivalent
- Certified home health aide by the State Board of Nursing
- Completion of state-approved certified nurse assistant course and test
- One year of experience in patient care in hospital, nursing home, or related facility
- Current health requirements as requested by Community Hospice
- Continuing education requirements per state licensure/certification
- Effective communication skills with patients, families, and health-care team
- Ability to work independently of direct on-site supervision
- Ability to participate in Federal and State Healthcare Programs
- Able to pass Driving Records Check, Criminal Background Check, and Pre-Employment Drug Screen
