Job Summary
Provide school and community-based case management, skills training, and services to children and their families referred from schools, the Department of Child Services, Juvenile Probation, and System of Care. Coordinate patient transfers and share information between State Psychiatric Hospital settings and Community Treatment Facilities to ensure continuity of care. Maintain appropriate records of services, document treatment plans, and submit reports according to established time frames. Communicate professionally with community agencies to make referrals and coordinate services. This full-time role requires a Bachelor's degree in mental health or human services, two years of experience with long-term psychiatric patients, crisis management training, and a valid Indiana driver's license.
Required Qualifications
- Bachelor's degree in a mental health or humans services related field
- Minimum of two years of experience working with long-term psychiatric patients
- At least 21 years of age
- Valid Indiana driver's license
- Valid car insurance
- Crisis management training
