State Psychiatric Hospital (SPH) Liaison

Coordinate patient transfers and ensure continuity of care for adults with serious mental illness by serving as the key connection between State Psychiatric Hospital settings and Community Treatment Facilities. Provide school and community-based case management, skills training, and referrals to children and families referred from schools, child services, probation, and assessment teams. Maintain appropriate records of services, document treatment plans, and submit reports according to established time frames while communicating professionally with community agencies. Hold a Bachelor's degree in mental health or human services with two years of experience working with long-term psychiatric patients, plus crisis management and verbal deescalation training.
