Job Summary
Conduct initial patient assessments and develop individualized care plans for new home health clients in Suffolk and Nassau County. Complete all required documentation, including OASIS entries, and educate patients and families on care procedures and medication management. Coordinate with interdisciplinary teams to ensure seamless transitions and maintain accurate records in compliance with state guidelines. Travel to assigned patient homes to initiate services and support continuity of care. This role offers flexible scheduling and includes orientation, training, and referral bonuses for Always Compassionate Health, a leading provider of community-based healthcare in New York.
Required Qualifications
- Current and valid RN license in New York State
- Strong knowledge of OASIS documentation and regulatory requirements
- Excellent organizational and communication skills
- Ability to travel to patient homes within assigned territory
