START Coordinator

Assess, coordinate, and facilitate formal and informal supports for Transition Aged Youth and their families to decrease crisis events and increase movement toward life goals. Conduct strength/needs/biopsychosocial assessments, develop individual treatment plans, and provide group and individual therapy as indicated. Coach clients and families in developing skills necessary to meet treatment goals while meeting regularly at sites, homes, or in the field. Provide prompt intervention in crisis events and assist in establishing client eligibility for Medi-Cal or other benefits. Complete required paperwork, including submitting outcomes and incident data into the identified database, adhering to funder and certifying body standards. Develop and participate in interdisciplinary team meetings and make recommendations for improvement in clinical programs.
