Job Summary
Responsible for assisting members in meeting their expressed goals toward crisis resolution and maintaining wellness while living in the community. Additional support in areas of medication management, transportation, counseling, and advocacy will be provided as needed. Meets formal documentation and START Model fidelity requirements; coordinates care with internal teams and outside agencies; develops treatment plans; provides counseling, and supports eligibility for benefits.
Required Qualifications
- Master’s degree in Social Work, Psychology, or a related field
- two years of varied experience as a provider of mental health services
