Job Summary
STARS Connector within Agape's Powerlines Community Network: manages a caseload of students and serves as a trusted bridge among students, families, schools, and community resources. Applies Science of Hope and Human Flourishing frameworks to help students and families identify goals, develop pathways, and strengthen well-being. Responsibilities include assessing needs, coordinating health, mental wellness, housing, education, employment, and wraparound services, documenting case notes in CaseWorthy, and ensuring program quality and accurate reporting. Requires strong interpersonal, organizational, and time-management skills, ability to work independently and collaboratively with schools and community stakeholders, and flexibility to work evenings/weekends as needed. Must support data-informed practice and participate in duties such as recording data, reporting, and program coordination; summer camp support and engagement with multiple agencies are included. Must have a Bachelor's degree and demonstrated ability to work with diverse individuals, with preferred case management experience and knowledge of community development; strong communication, computer proficiency (Microsoft Office), problem-solving, and self-starting capabilities are desired. A valid automobile and driver's license with appropriate insurance are required, along with willingness to work a flexible schedule.
