Job Summary
The Service Coordinator III LVN is responsible for coordinating care and utilizing healthcare resources for STAR Kids program members. This role involves assessing member needs, facilitating care through communication with healthcare teams, and ensuring compliance with state and federal regulations. Key responsibilities include service coordination, linking members to services, enrolling in disease management, and performing quality assurance tasks. Candidates should possess a high school diploma with an Associate's degree preferred, current LVN licensure, and at least three years of clinical experience.
Required Qualifications
- High school diploma or equivalent
- Current, active, and unrestricted licensure as a Licensed Vocation Nurse (LVN)
- Minimum 3 years of clinical experience (pediatric preferred) or 1 year experience in managed care
- Previous service coordination or case management experience
Desired Qualifications
- Experience in a managed care environment
- Direct experience working with children and young adults with similar conditions
- Knowledge of the State of Texas STAR+PLUS program or similar programs dealing with SSI and disabled children
- Knowledge of Medicare/Medicaid dual population
Additional Requirements
