Job Summary
Staff Pharmacist in a Skilled Nursing Pharmacy responsible for dispensing medications accurately, conducting medication therapy management and patient counseling, collaborating with healthcare providers to optimize regimens, reviewing prescriptions for accuracy, monitoring patient outcomes, educating nursing staff on medication administration and safety, ensuring regulatory compliance and quality standards, and participating in ongoing professional development. Requires a PharmD, active WA license, and experience in clinical/skilled nursing settings; strong communication, detail orientation, and EHR/pharmacy software knowledge are essential.
Required Qualifications
- PharmD degree from an accredited pharmacy school
- Active Washington State pharmacist license
- Minimum of 1 year experience in a clinical or skilled nursing setting preferred
- Strong communication and interpersonal skills
- Detail-oriented with a commitment to patient safety and quality care
- Ability to manage multiple tasks in a fast-paced environment
- Knowledge of pharmacy software and electronic health records
