Job Summary
Monitor, modify, and evaluate drug therapy for optimal therapeutic outcomes while interacting with patients to obtain drug history, counsel them on side effects, and explain current regimens. Perform pharmaceutical compounding for sterile and non-sterile preparations, screen prescriptions for appropriateness, and evaluate medication orders against clinical guidelines and patient-specific factors. Dispense medications with attention to accuracy and regulatory compliance, then review clinical data to assess efficacy and toxicity. Participate in medication reconciliation, interdisciplinary rounds, and collaborative treatment planning to advise physicians and nurses on dosage and adverse effects. Provide individualized counseling, conduct educational programs, and support quality improvement initiatives including adverse event tracking and formulary management. Oversee pharmacy technicians to ensure compliance with legal requirements and proper storage procedures.
Required Qualifications
- Valid, unrestricted insurable driver's license
- Resumes and references
- Completion of an accredited college or university with a Bachelor's degree in Pharmacy or Pharm.D.
- Active, unrestricted Pharmacy License in any U.S. State or Territory
- Completed training program as defined in the education qualifications listed below
