Job Summary
Process patients from assessment to discharge in medication-assisted and drug-free treatment programming. Dispense all medication per established procedures and monitor methadone dosages. Monitor the physical and mental health well-being of patients, documenting findings regularly. Be available to travel if needed. The role involves working within a non-traditional LPN setting at UIH Rockford, Monday through Friday with every other Saturday, starting ASAP.
Required Qualifications
- Must be available to travel if needed
- DCFS clearance required
- L.P.N. registered in the State of Illinois
- certified in CPR
Desired Qualifications
- At least two years' experience in hospital or agency setting preferred
