Job Summary
The Medically Assisted Treatment Case Manager/Staff Nurse role supports substance use treatment services within CHC’s multi-site health center. Responsibilities include program oversight and administrative support, serving as the point of contact with DPH for Suboxone, Naloxone, and Vivitrol treatment services across CHC, ensuring data is collected and entered into the appropriate systems, and following up with patients to coordinate appointments. The position collaborates with the CMO, ACTION & Behavioral Health Director, and COO, and requires handling highly sensitive medical and behavioral health information with strict confidentiality. Skills emphasized include decision-making, problem solving, attention to detail, effective communication (oral and written materials, reports, memoranda), and the ability to develop solutions and recommendations with guidance from superiors in grant and contract management. The role is full-time and domiciled on-site at CHC’s facilities in Fitchburg, Leominster, and Gardner, MA, with comprehensive benefits and professional development opportunities.
Required Qualifications
- 2-year college degree in hospital, public or business administration, health or human services or related disciplines; 4-year degree preferred, or associates degree; licensed as a RN in Massachusetts
- 12-24 months of case management experience along the care continuum
- AHA BLS Health Care Provider certification or equivalent
