Staff Nurse (RN OR LPN) - Leominster

Job Summary
Conduct comprehensive, patient-centered nursing care in an outpatient primary care setting, including vital signs, medication administration, wound care, and immunizations. Monitor patients for acute and chronic conditions while coordinating referrals, diagnostic testing, and post-discharge follow-up. Provide patient education on diagnoses, lifestyle changes, and preventive care to promote adherence and health literacy. Accurately document assessments and interventions in the electronic health record while maintaining compliance with regulatory requirements. Collaborate with physicians, advanced practice providers, and the interdisciplinary team to ensure safe, efficient care delivery. Work at least one evening per week in a fast-paced medical office environment with no direct supervision.
Required Qualifications
- Registered Nurse or License Practical Nurse License
- Work experience equivalent to one year as an RN/LPN in an ambulatory medical office environment
- Currently registered as a Registered Nurse or Licensed Practical Nurse in the Commonwealth of Massachusetts and its Board of Registration in Nursing
- AHA BLS Health Care Provider certification or equivalent
- Required to work at least one evening per week
- Demonstrated interpersonal relationship skills
- Demonstrated written and verbal communication skills in English
- Demonstrated ability to work in a fast-paced medical office environment
- Demonstrates understanding and commitment to the health center mission
- Demonstrates understanding and commitment to the established CHC Values and Standards
Desired Qualifications
- Bilingual in Spanish and/or Portuguese is a plus
- Work experience equivalent to one year as an RN/LPN in an ambulatory medical office environment preferred
- Computer skills for accurate data entry
