Job Summary
Monitor the health status of individuals with intellectual and developmental disabilities, triage urgent care concerns, and administer medications. Report changing conditions to team members and arrange medical services while documenting all assessments per regulatory standards. Teach staff and families on health promotion, self-responsibility, and disease prevention. Provide hands-on training for direct care workers on vital signs, medication administration, and safe use of medical devices like CPAP machines and lifts. Respond to emergency calls on a rotating basis and serve as backup nursing support when assigned nurses are unavailable. Advocate for the needs of developmentally disabled persons and collaborate with physicians and support coordinators to ensure continuity of care.
Required Qualifications
- Licensed as a Licensed Practical Nurse (LPN) or Registered Nurse (RN) in good standing with the State of Minnesota
- Demonstrated proficiency in managing health services for people who have intellectual disabilities and other related conditions
- Valid driver's license and driving record that meet the requirements of Hammer Residences insurance carrier
- Must receive a MN DHS Background study clearance and show a negative Tuberculosis test result
- Excellent written and verbal communication skills including fluency in English
- Proficiency with Microsoft Office (especially Outlook and Excel) and SharePoint
- Strong organizational skills

- Ability to push or pull an individual in a wheelchair or while assisting with cares
- Ability to perform twisting/turning, kneeling/squatting, reaching, and assisting with pivot transfers
- Ability to operate specialized medical or adaptive equipment, including use of lifting equipment and driving a lift van
- Must be willing and able to work flexible hours/days/occasional evenings or weekends
- Responsible for emergency call rotation
