Stability Services Case Manager

Job Summary
Maintain a caseload of 35-45 clients by providing intensive, ongoing in-home case management support to formerly homeless individuals and families. Conduct monthly case management meetings in clients' homes and communities while performing assets/need assessments to develop Housing Plans and Safety Plans. Complete core Center For Working Families assessments, assist clients in applying for mainstream benefits including MDHHS and Section 8, and foster inter-agency collaborative solutions to coordinate referrals. Enter data in real-time or within 48 hours using HMIS and Salesforce to track case management activities and client outcomes. Assist with front-desk operations, answering phones, and crisis calls as needed.

- Proficiency with Microsoft Office Suite
Desired Qualifications
- 1-2 years experience in human services
- Experience with HMIS
- Experience with Google Apps
