Job Summary
Receive referrals from Coordinated Entry and screen clients for Rapid Resolution using standardized vulnerability assessments. Complete intakes with referred clients to determine eligibility, assess housing needs, and develop goal-oriented Housing Stability Plans. Assist clients in locating and securing permanent housing while navigating them to community resources and utilizing VA and local services. Enter client data into the Homeless Management Information System (HMIS) and maintain accurate records and reports. Update and carry out client plans while attending weekly Navigator meetings as necessary. Provide crisis intervention and treat clients in a culturally sensitive manner throughout the process.
Required Qualifications
- Post-secondary education in a social services field
- Minimum of 1 year experience working with individuals and/or families with complex needs
- Experience providing services to low-income/no-income households
- Knowledge of and ability to access community housing, social, medical, mental, chemical dependency services and other needed resources
- Ability and willingness to work independently as well as in a team environment
- Highly motivated self-starter
- Ability to coordinate multiple projects/tasks simultaneously in a high-pressure environment
- Excellent interpersonal skills including excellent oral and written communication skills
- Excellent organizational and time management skills
- Ability to maintain accurate, thorough, timely and complete client files
- Computer skills including Microsoft Word and database experience
- Ability to work with a diverse spectrum of high-need individuals
- Reliable personal transportation
- Valid Washington driver's license
- Auto insurance
- Acceptable driving record according to CCS' driving policy
- Willingness to provide current driving abstract
- Willingness to obtain updated tuberculosis test upon employment
- Willingness to obtain CPR, First Aid training upon employment
