SSD

Plan, develop, organize, implement, evaluate, and direct the overall operations of the Social Services Department. Assist with facility admissions, manage resident trust accounts, and coordinate resident shopping. Schedule resident appointments and maintain accurate charts and documentation of resident care. Engage and converse with residents to promote a positive environment. Work primarily during daytime hours, with occasional evening and weekend visits as required. Prefer 2-5 years of Social Work experience in a Long Term Care setting, including at least 1 year of supervised social work experience in health care. Knowledge of Medicaid and Medicare programs is a plus.
