Job Summary
Acts as the primary point of coordination for eligible individuals, assesses wants and needs, develops the Individual Service Plan (ISP), and coordinates services and supports to ensure health and welfare. Responsibilities include collaborating with families, guardians, waiver service providers, and community members; conducting follow-up discussions via phone, email, video, in-person or other contact; ensuring services are within the assigned ISP budget; developing, monitoring, and reviewing Restrictive Measures as required; following up with providers to ensure service delivery; monitoring progress toward ISP outcomes; ensuring Level of Care Assessments and other Medicaid/County requirements are completed; connecting individuals with appropriate community resources; educating individuals on provider choice; documenting services with TCM notes; adhering to timelines and policies; providing crisis assistance and ensuring Medicaid eligibility; and training providers on ISPs while advocating for technical assistance. An integral part of the role is ensuring services and oversight focus on the individual served, with a strong emphasis on person-centered planning and collaboration across multiple parties.
Required Qualifications
- Bachelor’s Degree in related field
- Experience with individuals who have developmental disabilities
- Eligible for DODD Certification
