Job Summary
Social Worker for the Short Term Residential (SRT) program provides assessment, discharge planning, case management, and community linkage services for adults in residential behavioral health treatment. Responsibilities include evaluating client and family needs, participating in weekly multidisciplinary treatment team meetings, developing and coordinating individualized treatment goals, conducting weekly therapeutic activities, and supporting clients through the residential treatment process; develop comprehensive discharge plans, link clients with community resources, coordinate with nursing/medical staff, document progress toward community transition; maintain comprehensive discharge plans and documentation; engage in training and professional development; perform duties in a residential behavioral health treatment environment with travel as needed. Location: Delray Beach, FL; Schedule: Full-Time; requires background check; EOE.
Required Qualifications
- Bachelor’s Degree in Psychology, Social Work, or a related field from an accredited college or university
- Minimum one (1) year of experience working with chronically mentally ill, dually diagnosed, or emotionally disturbed adults

- Strong clinical assessment and case management skills

- Two (2) or more years of experience working with chronically mentally ill, dually diagnosed, or emotionally disturbed adults
- Experience in residential, inpatient, or behavioral health treatment settings
- Knowledge of community resources, housing services, and discharge planning processes
