Job Summary
Provide case management for chronically homeless individuals and families within the Supported Housing Program. Develop and implement a Housing Health and Recovery Plan focused on person-centered, strength-based recovery; secure apartments in affordable housing; address life domains including employment, finance, relationships, medical needs, legal concerns, education, transportation, safety, culture, spirituality, and social aspects; collaborate with recipients to integrate into a chosen community; plan and facilitate discharge when ready, ensuring linkages to supports; model guidance for SHCM staff; respond to crises; complete required documentation; comply with NYS OMH regulations; participate in flexible scheduling including evenings and weekends; on-call and emergency response duties.
Required Qualifications
- BA required
- 2 years of experience working in case management or mental health services and/or equivalent experience
- Minimum 1 year experience with Housing.
