Job Summary
The Registered Nurse Senior Case Manager coordinates patient care across the continuum, integrating discharge planning, utilization management, and community resources to ensure safe, timely, and cost-effective transitions of care. Responsibilities include developing and implementing discharge plans, collaborating with physicians and multidisciplinary teams, co-managing caseloads with Social Worker Case Managers, addressing social determinants of health, and leading discharge processes to optimize length of stay and patient outcomes. Proficiency with CMSA standards, EHRs, and clinical decision tools (e.g., MCG, InterQual) is expected; role may serve as a preceptor and lead trainer for new hires. Certification and licensure requirements in Virginia are specified.
Required Qualifications
- Bachelor of Science in Nursing (BSN)
- Master’s degree preferred
- Active RN Licensure for the state of Virginia
- CPR certification within 6 months
- Case Management Certification through ACMA or CCM
