Assess the nutritional status of patients, translate physician-ordered diets to meet individual needs, and develop care plans for inpatients and outpatients. Coordinate with clinical staff to create nutritional plans by reviewing records and assessing the best course of action for nutrition therapy. Identify potential problems through direct observation or data analysis, institute corrective action, and individualize care plans to address patient safety, mobility, and specialized dietary requirements. Communicate verbally, in writing, and through databases with the healthcare team regarding nutritional needs, risks, and patient complaints. Perform counseling and nutritional health teaching, ensuring compliance with hospital policies and regulatory agencies.
