SR PHYSICAL THERAPIST

Conduct independent clinical practice including evaluation, treatment planning, and goal setting for physical therapy patients. Perform initial, progress, and discharge evaluations while establishing treatment plans based on evidence-based practice. Coordinate with medical personnel regarding patient progress and discharge planning, and involve patients and families in education and goal setting. Maintain and update patient documentation in accordance with professional, departmental, and payer guidelines. Participate in program development, clinical coordination of students, and continuing education requirements. Serve as a resource person for staff, provide mentorship, and act as a clinical instructor for students.
