Job Summary
Senior Case Manager/Case Manager at ACMH (St. Hilda’s House) in Manhattan provides case coordination and delivers community rehabilitation services. Responsibilities include conducting home visits, obtaining psychosocial histories, liaising with primary psychiatric care providers, ensuring access to psychiatric, substance use, medical services and entitlements, monitoring medications, administering standardized health screenings, developing individualized rehabilitation plans, delivering community rehabilitation and housing support services, documenting client progress, training clients in safety procedures, preparing residents for independent living and facilitating crisis intervention. Also responsible for maintaining client charts, incident reporting, supporting maintenance needs, staff coverage, and serving as Qualified Mental Health Staff Person. Required qualifications include a Master’s degree in psychology or a related human-service field (or BA with 3+ years direct care experience), strong communication skills, and experience with adults with serious mental illness. Salary ranges for Single Site SCM and CM are $60,355 and $52,081 respectively, with generous benefits.
Required Qualifications
- M.A. in psychology or a human service related field
- B.A. in psychology or a human service related field with a minimum of 3 years of direct care experience
- experience with adults with serious mental illness preferred
- strong written and verbal communication skills
- Master’s Degree preferred
